Credential Program Applications & Forms

 

PLEASE NOTE:

Fall 2010

  • The priority application filing period for Fall 2010 credential programs ends NOVEMBER 30, 2009.  Applications submitted between December 1, 2009 - March 1, 2010 will only be reviewed if space becomes available.

  • Applicants who have not passed all sections of the CSET Exams must submit verification they are registered for the November exam date. Applicants that have not taken the CSET by the November exam date will not be eligible to apply for the Fall 2010 semester.

  • Applicants who have not passed the CBEST Exam must submit verification they have registered no later than the December exam date. Applicants who have not taken the CBEST by the December exam date will not be eligible to apply for the Fall 2010 semester.

Subject Matter Competence Verification (SMCV)
This form is only for applicants completing their coursework at Sacramento State.

 

NOTE: The application forms are available in both PDF format (requiring the free Adobe Acrobat Reader) and Microsoft Word format. You may type your information into the MS Word version of the application, then print it. The PDF format requires that you print the application, then fill it out.
