Nomination Instructions

You may nominate one or more individuals, businesses, organizations, or groups. 

To make a nomination, follow these steps:

  1. Read the nomination criteria.

  2. Collect the information to support your nomination and submit either online or via a paper forms.

  3. Submit the forms. The online form is submitted by simply clicking the "Submit Nomination" button at the bottom of the form. Paper forms can be emailed, faxed, or mailed. See the instructions. Please note the nomination deadline.
