Forms will not be accepted without all appropriate signatures and studnet ID number (not SS#).
All students must meet with their assigned advisors at least once a semester and keep an up-to-date program advisement form on file and available.

Should be completed as soon as the student has: Cleared all conditions, including WPE, through the department (ie., full classification); and Completed at least 24 units of 200-level coursework towards the MA with a 3.0 GPA or better; Must be advanced to candidacy in ordder to register for EDS 249, 540, and 541.
